You've come to the right spot. If your child will be five years old by August 1, 2024 and your student lives in Slade's boundary area , start the process by clicking here and filling out the form. Then come by the office between 7:30 am and 3:30 pm with a copy of your child’s state certified birth certificate and a current immunization record.
